Where your previous cases validated by your former school? If not ,you will have a hard time obtaining the necessary signatures. 
For 2011 graduates of CMO # 14  , you need to accomplish the new forms . 
Please search in the BON website the memo regarding this matter.

		<description>good day mam anna..i need your professional opinion on this one..just want to know if there is an expiration date for the cases (O.R, D.R)...i&#039;ve been hearing a lot of rumors regarding this matter and i&#039;m worried because i stopped nursing school a couple years ago but decided to pursue recently. all my cases were from the same school but were dated 4 years ago..is there such rule from PRC or CHED regarding this matter? your reply will be greatly appreciated..thank you so much and God Bless...</description>

If you are into the CMO #14 curriculum,It will be a lot easier for you, (lessen the hazzle) if you get new cases, because the forms are different. Consult your clinical coordinator of your new school and ask about your case. Thanks.</description>
